This privacy policy sets out how we use and protects any information that you give when you become a client of Em-powering Change.

We are committed to ensuring that your privacy is protected. Should we ask you to provide certain information by which you can be identified, then you can be assured that it will only be used in accordance with this privacy statement.

Em-powering Change may change this policy from time to time. A copy of this policy will be available in the office for you to look at.  This policy is effective from 23 May 2018.

What we collect

As a client, depending on the nature of our working relationship we may collect some or all of the following:

  • Name
  • Date of Birth
  • Dr’s Name and any medication
  • Emergency Contact Name
  • Job title
  • Contact information including address, email address and telephone number
  • Summary of the reason for contact
  • Your contact preferences
  • Notes of meetings

What we do with the information we gather

We require this information to understand your needs and provide you with a better service, and in particular for the following reasons:

  • Internal record keeping.
  • Agreeing, altering appointments.
  • We may use the information to improve our products and services.
  • We may periodically send promotional emails about new products, special offers or other information which we think you may find interesting using the email address which you have provided.
  • From time to time, we may also use your information to contact you for market research purposes. We may contact you by email, phone or messenger depending on your contact preferences.

Security

We are committed to ensuring that your information is secure. In order to prevent unauthorised access or disclosure, any computerised files are password protected on a computer protected by a separate password.  Any written notes of coaching sessions will be locked in a filing cabinet.  If you have made notes as part of our session, we may ask to take a photo for our records.  We will print this out, file it and delete the photo from our phone.

For insurance purposes, we will need to retain records for 7 years from the end of our last contact.  In the case of children, this is 7 years from their 18th birthday.  At the end of this time, the records will be shredded.
